Addendum to Consent - Oily Skin Release

You will be asked if you have read this when signed in before your procedure.

By reading this I am understanding that a combination/oily or severely oily skin can/will cause the pigments to:
fade prematurely, look ashy or gray, appear more powdered looking or not retain at all.
Despite Atom’s recommendation, I wish to proceed and take full responsibility in accepting these risks and will NOT HOLD Atom, his business and/or his associates responsible if said risks occur. I take full responsibility and accept that this will/could occur.
All services performed will be non-refundable and final.

I am aware that if I have any questions I need to consult with my artist before proceeding with any procedures.
