Photography Release Consent
You will be asked if you have read this when signing in before your procedure.

“Before” and “After” photos will be taken and kept on file. We would like your permission to use these photos for advertising, portfolios, online and in-print ads, education purposes, etcetera.
If you do not consent to the use of these photographs, please advise your artist to which are to not be used for the purposes mentioned;
(Eyebrows) Full Face___ Eyebrows and Eyes___
(Eyeliner) Full Face___ Eyes Only___
(Lips) Full Face___ Lips Only___

I am aware that if I have any questions I need to consult with my artist before proceeding with any procedures.
