Consent and Release Agreement
This form is designed to give information needed to make an informed choice of whether or not to undergo a permanent cosmetics application. If you have questions, please feel free to ask. Although permanent cosmetic tattooing is effective in most cases, no guarantee can be made that a specific client will benefit from the procedure. This is the process of inserting pigment into the dermal layer of the skin and is a form of tattooing. All instruments that enter the skin or come in contact with body fluids are sealed and sterilized before use and disposed of after use. Cross contamination guidelines are strictly adhered to. Generally, the results are excellent, however, a perfect result is not a realistic expectation. Usually, a touch-up is expected after the healing is completed.

Initially, the color will appear much more vibrant or darker compared to the end result. Usually within 5-7 days, the color will fade 10-50%, soften and look more natural. The pigment is permanent but will fade somewhat over time and will likely need to be touched-up through the years. Possible risks, hazards or complications:
• Pain: There can be pain even after the topical anesthetic has been used. Anesthetics work better on some people than others.
• Infection: Infection is very unusual. The areas treated must be kept clean and only freshly cleaned hands should touch the areas. Click HERE for aftercare instructions
• Uneven Pigmentation: This can result from poor healing, infection, bleeding or many other causes. Your follow up appointment will likely correct any uneven appearance.
• Asymmetry: Every effort will be made to avoid asymmetry but our faces are not symmetrical, so adjustments may be needed during the follow up session to correct any unevenness.
• Excessive Swelling or Bruising: Some people bruise and swell more than others. Ice packs may help and the bruising and swelling typically disappears within 1-5 days. Some people don’t bruise or swell at all.
• Eye Exposure: There is a small risk of eye injury when an eyeliner procedure is performed. To avoid corneal abrasion, Celluvisc, a thick eye drop is used to protect the eye prior to the procedure. Eye drops are used to cleanse and flush the eye after the procedure is complete.
• Anesthesia: Topical anesthetics are used to numb the area to be tattooed. Lidocaine, Prilocaine, Benzocaine, Tetracaine and Epinephrine in a cream or gel form are typically used. If you are allergic to any of these please inform me immediately.
• MRI: Because pigments used in permanent cosmetic procedures contain inert oxides, a low level magnet may be required if you need to be scanned by an MRI machine. You must inform your technician of any tattoos or permanent cosmetics you may have.
• Fitzpatrick skin types 1-2 (Red heads/Blondes-fair skin-light eyes). You will be red, swollen and sometimes the pigment will not retain well and you may require additional procedures.
• Fitzpatrick skin types 3-6 please note the hair strokes will not be as visible as on lighter skin types.
• Alopecia - Due to change in skin & follicles, not all types of Alopecia clients are candidate for hair strokes technique.
• Large Pores/Oily/Severely Oily Skin (Pigment will look more powdered/solid)
• Have Thyroid/Graves disease, Hepatitis (A,B,C) Medications cause the pigment not to retain properly.
• Trichotillomania-pulling disorder - Due to scar tissue pigment will not retain well & WILL blur.
• Eczema or Dermatitis in or around the eyebrows (chronic shedding, bumps, redness, irritation or flaking of the skin in the eyebrow area) will cause pigment not to retain properly.
• Any type of organ transplant that requires anti-rejection medications will cause pigment not to retain properly.
• I understand that pigments implanted in TAN skin WILL potentially heal darker, appear ashy and WILL NOT look like hair strokes. Tan skin may also result in a more faded appearance due the skin having been damaged prior to the procedure.
• I understand there is a small possibility of an allergic reaction. You may request a 15 min patch test to determine this.
• The alternative to these possibilities is to use cosmetics and not undergo the Permanent Cosmetics procedure.

By signing the waiver in the app provided by Forbidden Fruit Tattoo & Piercing, I agree to the following:
• I understand if I wear contact lenses and I am receiving a procedure concerning my brows or eye liner, it is my responsibility to either wear glasses or bring a contact case with me. I may also bring my own solution or drops for flushing my eyes which will likely occur multiple times during the eye liner procedure.
• I understand that being on any acne medication will cause pigment NOT to stay.
• I understand I need to be off any steroid medication for one month prior to procedure or pigment WILL NOT stay.
• I understand aftercare instructions will be explained to me at the end of the procedure and a copy will be given to me to retain in my possession, which I will follow. Failing to do will increase the likelihood of a bad heal and may have adverse reactions. I understand I have access to the aftercare instructions located on this site located HERE.
• I understand that a certain amount of discomfort is associated with this procedure and that swelling, redness and bruising may occur.
• I understand that Retin A, Renova, Alpha Hydroxy and Glycolic Acids must not be used on the treated areas. I am aware that using these products will alter the color.
• I understand that sun, tanning beds, pools, some skin care products and medications can affect my permanent makeup. I will tell all skin care professionals or medical personnel of my permanent make-up procedures, especially if I’m scheduled for an MRI.
• I accept the responsibility to explain to you my desire for specific colors, shape and position for any procedure done. I will also provide any references including photographs of my face and personal cosmetic practices to better convey my needs.
• I understand that implanted pigment color can slightly change or fade over time due to circumstances beyond your control and I will need to maintain the color with future procedures, potential touch-up session within 60 days, and traditional cosmetics.
• I acknowledge that the proposed procedure(s) involve risks inherent in the procedure and have possibilities of complications during and/or following the procedures such as: infection, misplaced pigment, poor color and retention and hyper-pigmentation.
• I have been quoted the cost of one permanent makeup procedure. This is for one session only. A 6-8 week touch-up perfecting session is recommended at an additional cost.
• I understand that I will contact my artist if signs/symptoms of infection arise and schedule a consult for assessment immediately. If suggested, I understand I may have to consult a physician if any signs and symptoms of infection such as fever, swelling, redness or drainage occur to acquire medical advice.
• I understand that if I do not follow the aftercare 100% I can ruin the whole process and will have to pay accordingly to repair the damages.
• I am NOT PREGNANT.
• I DO NOT HAVE DIABETES. If I do, I understand that any adverse reactions or poor healing due to my Diabetes I assume all risk therein.
• I am not intoxicated or under the influence of drugs or alcohol.
• I understand and agree that Atom Godwin/Forbidden Fruit Tattoo & Piercing is not responsible for any adverse reactions I may experience from the Covid-19 vaccines. For more information on the C19 Vaccines vs PMU, click HERE.
• I acknowledge a $100.00 non-refundable liquidated damage fee (deposit) will be charged to reserve spot for an appointment, and will be forfeit due to a no-call/no-show situation.
• I understand this type of tattooing typically takes 4-8 weeks to heal completely. I do hereby release and forever discharge “Atom Godwin/Forbidden Fruit Tattoo & Piercing” from any and all actions, causes of actions, claims and demands for, upon or by reason of any damage, loss or injury, which hereafter may be sustained by having this procedure performed.
• I understand that Atom Godwin/Forbidden Fruit Tattoo & Piercing can NOT guarantee the results for corrective/coverup work. If the previous tattoo is too dark, large, discolored, tattoo removal is recommended before booking an appointment for any new cosmetic tattoo procedures to achieve the best results. • I understand that corrective/coverup work will generally require multiple sessions for best results and additional touch ups will be charged accordingly.
• I understand that during a permanent makeup/cosmetic tattoo procedure there is a risk that pigment/ink may stain my clothes and it is my responsibility to dress accordingly. I accept that and will not hold Atom Godwin/Forbidden Tattoo & Piercing responsible for any damages.
• I take full responsibility for RELEASE AND HOLD HARMLESS (Atom Godwin/Forbidden Fruit Tattoo & Piercing), the owner, from any and all liability, claims, demands or causes of action that I may hereafter have for injuries or damages arising out of this procedure.
• I further agree that I WILL NOT SUE OR MAKE CLAIM against Atom Godwin/Forbidden Fruit Tattoo & Piercing, the owner, for permanent makeup performed on me. I also agree to INDEMNIFY AND HOLD THE RELEASED PARTIES HARMLESS from all claims, judgments and costs including attorney’s fees, incurred in connection with any action brought as a result of having said procedure done by Atom Godwin/Forbidden Fruit Tattoo & Piercing.
• I further represent that I am at least 18 years of age, I waive and release any and all legal rights that may accrue to me as the result of any injury I may suffer while engaging in permanent tattooing by Atom Godwin/Forbidden Fruit Tattoo Piercing.
• I certify that I have read or have had the contents of this form read to me. I understand the risks and alternatives involved in this procedure(s) and I have had the opportunity to ask questions and all of my concerns have been answered. I acknowledge that I have reviewed and approved the materials given to me and I authorize Atom Godwin/Forbidden Fruit Tattoo & Piercing as my permanent cosmetic technician to perform on my body and face the procedures I have chosen.

I am aware that if I have any questions I need to consult with my artist before proceeding with any procedures.
