Addendum to Consent - Thyroid Release

You will be asked if you have read this when signed in before your procedure.

By reading this I am understanding that Thyroid medicines can and may cause the pigments to:
fade prematurely, look more blurred or powdered under the skin, change in color or not retain at all.
Atom gave me a full explanation of the process and I wish to proceed and accept all risks and will NOT HOLD Atom, his business and/or his associates, responsible if said risks occur. Appointments will not be made any sooner if the above mentioned issues occur.
All services performed will be non-refundable and final.

I am aware that if I have any questions I need to consult with my artist before proceeding with any procedures.
